      Step 1- Option 1 or 2 

      1. Getting onto NDIS - $800
      2. FCA $2,000 report depending on complexity

      (please confirm signatory with the referrer- if not participant get details immediately and make contact ) 

       

      Step 2 

      Book in appointment with Peter (OT) on the phone instantly, get basic information about participant 

       

      Step 3

      Make sure information about participant is available to OT prior to initial appointment commencing. (Background details about participant, past medical history)

       

      Step 4 

      Paperwork to be signed at assessment
